Step One:

Step Two:
Either print the file or use a free pdf software like DocHub or Google Docs to fill out form.
Call Larry. Larry has done this for years and can make the sheet much easier to fill out.
Step Three:
Scan/photograph sheet or email the filled out pdf back to the Wilson’s.
